Become a Subsidy Provider

Providers must register with the Child Care Subsidy Program before they can be reimbursed for subsidized child care services. The following types of Child Care Providers are eligible:

Licensed child care centers or homes;

Non-licensed family, friend or neighbor;

License-exempt recreation programs

The following persons are not eligible to be Subsidy program providers:

  • The natural/adoptive parent or guardian of the child;
  • Anyone living in the same residence as the child;
  • Anyone who is a parent on their own subsidy case;
  • Anyone who fail the mandatory criminal background check

Background Checks & Monitoring

All providers must complete a comprehensive background check of all staff and adults over 18 with access to the children in care. The comprehensive background check includes:

  • FBI Fingerprint check; and
  • State of Nevada Criminal Repository check; and
  • State of Nevada Sex Offender Registry check; and
  • State of Nevada Child Abuse and Neglect Registry check.

Individuals who do not pass the mandatory background check are ineligible to be a Subsidy provider. For a list of disqualifying crimes that prevent approval, click here.

FFN Providers

Registration Submission:

Fax completed registration documents to 702-410-9909 or email them to [email protected]. A Child Care Resource & Referral Specialist will review the documents and schedule a registration appointment.

Required Packet Documents

  • Provider Registration Form
  • Provider Payment Option Form
  • W-9 Request for Taxpayer Identification
  • Provider Service Agreement or Memorandum of Agreement (MOA)

Additional Required Documents

  • Copy of current Child Care License
  • Rate Sheet with all fees charged to families
  • Copy of current Parent Handbook
  • Proof of age (18 or over)
  • Photo ID
  • Social Security Card and/or proof of US citizenship or Lawful Permanent Resident status
  • Working Emergency phone number
  • Verification of physical address separate from child needing care
  • Background Disclosure Form (must complete the check prior to approval)
  • Parent/Provider Agreement and Employer’s Responsibility Form
  • Certificates demonstrating required health and safety training is completed within 90 days of enrollment and then annually thereafter

General Training Requirements

All child care workers (licensed, non-licensed, and license exempt) must complete mandatory health and safety training to ensure children are in safe environments.

Training Requirements by Type

Licensed child care staff are required to complete 24 hours of Nevada Registry approved training within 120 days and annually thereafter.

Licensed child care staff are required to complete 24 hours of Nevada Registry approved training within 90 days and annually thereafter. The provider must submit a copy of training certificates demonstrating compliance.
